- We offer 8 designated patient parking spaces directly in front of our building
Arrival Instructions
To help us stay on schedule and provide the best care:
- Arrive on time
- New patients: Arrive 15 minutes early
- Bring your current insurance card and photo ID
- Bring a list of medications and allergies
- Masks are optional
- Minors: A parent/guardian must accompany the patient
- A “Consent to Treat a Minor” form may be completed for future visits — ask at check-in
